Bear River COVID-19 Vaccine Information


Bear River Local Health Department Vaccine Update 28 Dec 2020

Our first doses of vaccination are starting to arrive. We will begin
clinics for healthcare providers starting next week. These clinics are for
healthcare providers and staff *only.* At this time we are *not* able to
offer it to providers' or staff''s families. We will have clinics in both
Logan and Brigham City. Below are the times and address for the clinics.

Logan -- 655 E 1300 N, Logan, UT 84341
Monday, December 28th, 9 AM-7 PM
Tuesday, December 29th, 1 PM-7 PM
Thursday, December 31st, 9 AM-4 PM

Brigham City -- 817 Parker Ln, Brigham City, UT 84302
Monday, December 28th, 2 PM - 7 PM
Wednesday, December 30th, 1 PM-7 PM

Due to the limited amount of vaccines we receive each week the appointments
will be on a first-come-first-serve basis. When we are notified of
additional vaccines coming this way we will create additional clinics and
then notify you of the additional availability so you can let your staff
know.

When an appointment slot is gray that means the available slots have been
filled. If you do not see any available times that means you will have to
wait for the next round. We will notify you when additional vaccine and
appointments are made available. We do not have any guarantees but at this
point, we are expecting additional vaccines each week.

On the JotForm there is a place to upload a picture of your insurance card.
Be sure to include both front and back. If you are not able to upload a
picture bring your card with you to the clinic and we can copy it here.

The clinics will be done by *curbside*. When you arrive for your
appointment please follow the signs for COVID only and park in a stall;* remain
in your vehicle,* our staff will come to you.

I have attached the Bear River Health Department HIPPA Form, the Moderna
Vaccine Information Sheet, and the V-Safe Information sheet. Please share
these with your staff as well. These can also be accessed on the JotForm as
well if needed.

Please distribute this information to all of your staff as soon as possible
so they can register if they would like to. If you have any questions or
